Enrollment Documents

Below you will find the documents needed to complete our enrollment process. Thanks again for choosing the Chidren's Learning Center. We look forward to caring for, nurturing and teaching your young children.

Required Enrollment Forms

• Application For Enrollment (Accompanied by a $30.00 non-refundable processing fee)
• Medical Statement (ODJFS form required. Updates every 13 months)
• Enrollment & Health Information/Emergency Transportation Form
• Permission to Release Child/Roster Permission
• Video/Audio/Photo Waiver of Privacy
• Routine Trip Walk Permission Slip
• Step Up to Quality Program
• CACFP Enrollment
• Request for Administration of Medication (as needed)
• Medical Care Plan (Reviewed annually if applicable)

Parents Submit Copies (of the following:)

• Proof of Guardianship (if applicable)
• ODJFS Eligibility Document (if applicable)
• Class Schedule if SSU student
• Financial Aid Award Notification, if SSU student
• Work/School Schedule (if ODJFS)

Preadmission Interview Documents

• Tuition Contract
• Alumni Documentation, if applicable
• Volunteer Form
